(Abbreviated \u2013 Full version on application form)<\/em><\/p>\n\n\n\n <\/p>\n\n\n\n Constantiaberg Funeral Policies are underwritten by SAFRICAN Insurance Company, FSP No. 15123 – an Authorised Financial Services Provider.<\/p>\n\n\n\n <\/p>\n\n\n\n The Insurer pays cash to cover funeral costs according to the cover amount (reduced scale for younger children) on the death of a person listed in the policy. Payment can be made to any funeral director of the member\u2019s choice, or to the beneficiaries.<\/p>\n\n\n\n <\/p>\n\n\n\n \u2018Individual Member\u2019 and \u2018Member & Spouse\u2019 policies do not include children. A \u2018Single Parent\u2019 policy includes the member and children up to 21 years old (25 for unmarried students). A \u2018Family Plan\u2019 includes the member, spouse and children (as described above). Other close relatives of the member or spouse may be included as “Extended Family\u201d in the \u2018Member plus Nine\u2019 policy.<\/p>\n\n\n\n <\/p>\n\n\n\n After the death of the Main Member, the surviving spouse can take out a new policy based on the original entry date (i.e. at the original age category for premium purposes) – individual or family.<\/p>\n\n\n\n <\/p>\n\n\n\n Six months waiting period is applicable should death occur as a result of natural causes. Immediate cover for unnatural causes (accident). 12 months in the case of suicide.<\/p>\n\n\n\n <\/p>\n\n\n\n The premium will be determined by the chosen policy option, cover amount, and age category of the Main Member. Premiums will not increase when one moves into a higher age bracket. However, this is a group scheme, and the Insurer may adjust premiums or benefits according to the claims experience of the group.<\/p>\n\n\n\n <\/p>\n\n\n\n Premiums are payable monthly in advance to Constantiaberg Funeral Home – by Cash, EFT, Stop Order or Debit Order. The member enjoys cover as long as premiums are paid. The policy does not become \u2018paid up\u2019 and there is no savings component. A grace period of 30 days is allowed; but If two premiums are missed, the policy will lapse and all premiums and benefits will be forfeited.<\/p>\n\n\n\n <\/p>\n\n\n\n There is a 30 day “cooling off” period after the inception of the policy, during which time the member will be free to cancel the policy by advising us in writing, and be refunded premiums paid (less the risk portion). The policy may be cancelled at any time by the Member or Safrican, by written notification.<\/p>\n\n\n\n <\/p>\n\n\n\n Safrican (through Constantiaberg Funeral Home) will endeavor to settle a claim within 48 hours, from receipt of ALL claim documentation, provided all the claim procedure criteria have been met. They reserve the right to request further documentation or information as they may deem necessary to accurately assess a claim.
